We will have flu shots available to students and staff, Monday, October 26,2026 at 8:30 in the PL room.

Students must have this form completed, signed and turned in to the nurse before September 21, 2026 to participate. Forms are also available in the counselor's waiting room.

A copy of the insurance card would be greatly appreciated.
